2010-2011 Commitment Form

Kolbe Kids’ requires the following information before a child is allowed to attend school.  This information must be updated throughout the school year.

-          Completed Enrollment Packet

-          Well Health Statement with doctor’s signature

-          Current immunization record

-          Signed copies of the Discipline and Guidance Form

-          Copy of the child’s current health insurance coverage
